white-space: normal; -webkit-tap-highlight-color: rgba(26, 26, 26, 0.301961); text-indent: 5px; font-size: 18px;">三、招生入學(xué)條件 （一）入學(xué)錄取順序。 第一，學(xué)區(qū)內(nèi)兩證齊全。適齡兒童及其父母戶口在我校招生范圍內(nèi)，且戶主為其父母，家庭實(shí)際居住地（房產(chǎn)證所有人為其父母或適齡兒童本人）屬我校招生范圍內(nèi)，且已入住。 第二，學(xué)區(qū)內(nèi)僅有房產(chǎn)證。適齡兒童及其父母戶口在豐城市但不在我校招生范圍內(nèi)，家庭實(shí)際居住地（房產(chǎn)證所有人為其父母或適齡兒童本人）屬于我校招生范圍內(nèi)，且已入住。</div><div class="s4" style="font-family: -webkit-standard; white-space: normal; -webkit-tap-highlight-color: rgba(26, 26, 26, 0.301961); font-size: 18px;">第三，學(xué)區(qū)內(nèi)三代同堂有兩證。適齡兒童及其父母戶口在我校招生范圍，戶主為其祖父母或外祖父母，實(shí)際居住地（房產(chǎn)證所有人為其祖父母或外祖父母）同時(shí)屬于我校招生范圍內(nèi)，且適齡兒童及其父母在城區(qū)無房產(chǎn)。此類適齡兒童僅限父母為獨(dú)生子女。</div><div class="s4" style="font-family: -webkit-standard; white-space: normal; -webkit-tap-highlight-color: rgba(26, 26, 26, 0.301961); font-size: 18px;">第四，學(xué)區(qū)內(nèi)單位公房。適齡兒童僅有城區(qū)戶口，戶主為其父或母，適齡兒童及其父母在城區(qū)居住為其父母的單位公房，且單位公房在城區(qū)屬唯一房產(chǎn)，單位公房屬于我校招生范圍內(nèi)，由單位統(tǒng)一出具相關(guān)證明并辦理手續(xù)。 第五，學(xué)區(qū)內(nèi)小產(chǎn)權(quán)房。適齡兒童及其父母在城區(qū)購(gòu)買了小產(chǎn)權(quán)房，戶口已遷入小產(chǎn)權(quán)房地址并已入住，或此房產(chǎn)在城區(qū)屬其父母唯一房產(chǎn)并已入住，小產(chǎn)權(quán)房在我校招生范圍內(nèi)。</div><h3>第六，學(xué)區(qū)內(nèi)有戶籍無房產(chǎn)。適齡兒童、少年僅有城區(qū)戶口，戶主為其父或母，其父母在城區(qū)無房產(chǎn)，隨其父母住租（借）我校學(xué)區(qū)內(nèi)房三年以上的，提供住戶的水電或煤氣發(fā)票，經(jīng)核實(shí)，可在我校就讀。適齡兒童、少年僅有城區(qū)戶口，其父母在城區(qū)無房產(chǎn)，戶主為其直系親屬或其本人，落戶時(shí)間至少滿六年，與戶口掛靠人或委托監(jiān)護(hù)人在我校學(xué)區(qū)內(nèi)實(shí)際居住生活三年以上，經(jīng)核實(shí)，可在我校就讀；戶主為其本人，無實(shí)際監(jiān)護(hù)人監(jiān)護(hù)的，須到父母生活工作地就讀。</h3> <h3><span class="s8" style="white-space: normal; white-space: normal; -webkit-tap-highlight-color: rgba(26, 26, 26, 0.301961); font-size: 18px;">（五）有關(guān)說明 1.上述條款中涉及的房產(chǎn)權(quán)證為房產(chǎn)證或不動(dòng)產(chǎn)權(quán)證（含正式購(gòu)房合同及購(gòu)房契稅繳交票據(jù)），且權(quán)證性質(zhì)為住宅。商住樓、商鋪等非住宅、非直系親屬間共有的房產(chǎn)均不作為我校招生范圍的依據(jù)。上述條款涉及期限的終算時(shí)間截止到2019年7月31日。</div>2.上述條款中涉及的“已入住”的鑒定標(biāo)準(zhǔn)為居住地使用水、電或天然氣繳費(fèi)六個(gè)月以上的發(fā)票或相關(guān)收費(fèi)部門打印的六個(gè)月以上明細(xì)。上述條款涉及期限的終算時(shí)間截止到2019年7月31日。</h3>3.上述條款中涉及的“在城區(qū)無房”“城區(qū)唯一房產(chǎn)”，均須提交由住建部門出具的城區(qū)無房產(chǎn)證明查詢表，表格在辦理地址或教體局領(lǐng)取。辦理地址：市行政服務(wù)中心房產(chǎn)不動(dòng)產(chǎn)查詢窗口。</h3>4.上述條款中涉及的“租住證明”，指住建部門辦理的《房屋租賃證》或居住地所屬社區(qū)（村委）開具的三年以上租住證明，自2020年起只認(rèn)定住建部門辦理的《房屋租賃證》。</h3>5.上述條款中涉及的“勞動(dòng)合同書”，指市人力資源和社會(huì)保障局出具正式勞動(dòng)合同書。</h3>6.上述條款中涉及的“五險(xiǎn)一金相關(guān)憑證”，指社會(huì)保障局出具的個(gè)人對(duì)賬單。</h3> 四、生源認(rèn)定登記及入學(xué)時(shí)間 1.我校生源信息認(rèn)定登記日期：8月15日-20日。</h3>2.登記。凡符合我校學(xué)區(qū)范圍就讀條件的適齡兒童、少年家長(zhǎng)或其他法定監(jiān)護(hù)人根據(jù)我校招生公告要求，于8月15－20日內(nèi)攜帶其子女或被監(jiān)護(hù)人，持報(bào)名登記所需材料到學(xué)區(qū)范圍學(xué)校報(bào)名登記。應(yīng)攜帶的報(bào)名登記材料：①《豐城市實(shí)驗(yàn)小學(xué)2019年秋季招生生源認(rèn)定登記表》；②戶口簿原件和首頁(yè)、戶主及新生信息頁(yè)的復(fù)印件；③房屋產(chǎn)權(quán)證件（房產(chǎn)證或不動(dòng)產(chǎn)權(quán)證、正式購(gòu)房合同和購(gòu)房契稅繳交票據(jù)等）原件和復(fù)印件；④父母雙方身份證原件及復(fù)印件；⑤出生醫(yī)學(xué)證明；⑥兒童預(yù)防接種審核報(bào)告；⑦《戶口簿》不能反映父母子女關(guān)系的，須同時(shí)提供結(jié)婚證、出生證原件及復(fù)印件；⑧生源認(rèn)定條件中規(guī)定的其他相關(guān)材料等。</h3>3. 審核。我校招生工作人員將對(duì)新生上交的《豐城市實(shí)驗(yàn)小學(xué)2019年秋季招生生源認(rèn)定登記表》中所填信息進(jìn)行逐一核實(shí)、完善，并收齊相關(guān)材料存檔備查。</h3>4. 公示。我校將在8月26日前，分門別類在校門口張榜公布入學(xué)信息采集結(jié)果。</h3>5. 報(bào)到。經(jīng)公示確定的新生統(tǒng)一于9月1－2日到我校報(bào)到，按要求及報(bào)名相關(guān)程序辦理正式入學(xué)報(bào)名手續(xù)。 </h3>  </h3>附：豐城市實(shí)驗(yàn)小學(xué)學(xué)區(qū)范圍</h3><p class="s17" style="font-family: -webkit-standard;
